Documents Required:

1. Copy of ID proof of the father.
2. Copy of ID proof of the mother.
3. Copy of Discharge summary of newborn provided from AIIMS.

Steps to Apply (AIIMS Bilaspur):

1. Application form for Birth Certificate (To be only filled by the Mother or Father of the Child).
2. Copy of Aadhaar card of father.
3. Copy of Aadhaar card of mother.
4. Copy of Discharge copy of file issued by AIIMS.

Certificates can be obtained from 
OPD Registration

Days: Monday to Friday

Timings: 2 PM to 5 PM

 

 

Documents Required:

1. Copy of ID proof of the applicant (legal heir of the deceased).
2. Copy of ID proof of the deceased.
3. Copy of death notification provided from AIIMS.
4. Copy of PM report with original stamp from police station in case of MLC Death.

Formalities to Apply (AIIMS Bilaspur):

1. Application form for Death Certificate (to be filled by the legal heir).
2. Copy of ID proof (e.g., Aadhaar) of applicant.
3. Copy of ID proof of deceased.
4. Copy of Death notification provided from AIIMS.

1. Post Mortem report duly signed with original stamp (Doctor/Police Station).
2. Photocopy of ID proof of the Applicant (only legal heir).
3. Photocopy of ID proof of the deceased.

Instructions to obtain Death Certificate in MLC

1. All the documents mentioned above to be submitted in MRD.
2. Death Report (Form no 2) to be filled by the applicant as an informant.
3. Application to be obtain Death Certificate to be submitted with all the above documents to MS Office.
4. All the documents along with the application duly signed by MS to be forwarded to MRD.
