Steps to Apply (AIIMS Bilaspur):

1. Fill the application form.
2. Attach copy of Aadhaar card.
3. Submit at the designated counter.

Certificates can be obtained from 
OPD Registration

Days: Monday to Friday

Timings: 2 PM to 5 PM

1. Copy of ID proof of the applicant (legal heir / authorised person).
2. Copy of ID proof of the patient / subject.
3. Copy of notification / report provided from AIIMS.
4. Copy of relevant report with original stamp in case of MLC.

Formalities to Apply (AIIMS Bilaspur):

1. Application form to be filled by the authorised person / legal heir.
2. Copy of ID proof (e.g., Aadhaar) of applicant.
3. Copy of ID proof of patient / subject.
4. Copy of notification provided from AIIMS.

1. Report duly signed with original stamp (Doctor / Police Station).
2. Photocopy of ID proof of the Applicant (only authorised person).
3. Photocopy of ID proof of the patient / subject.

Instructions for Special Case

1. All documents mentioned above to be submitted in MRD.
2. Relevant Form to be filled by the applicant as an informant.
3. Application to be submitted with all the above documents to MS Office.
4. All documents along with the application duly signed by MS to be forwarded to MRD.
